What We Do

Committed Home Care is a partner to Public Partnerships LLC (PPL), the statewide Fiscal Intermediary for New York's Consumer Directed Personal Assistance Program (CDPAP). We support patients and caregivers throughout New York State, helping you enroll, onboard your caregiver, and stay compliant so care continues without disruption.

Our focus is support. We explain the process, help complete required forms, and assist with common issues so patients and caregivers can stay on track.

Facilitator Services

For Patients (Consumers)

  • Help you understand CDPAP eligibility and next steps
  • Guide you through enrollment and required documentation
  • Support with common questions and troubleshooting
  • Ongoing support once you are active in the program

For Caregivers (Personal Assistants)

  • Caregiver onboarding support and document collection
  • Assistance completing required forms and compliance steps
  • Help understanding timesheet submission requirements
  • Support if issues come up with onboarding or documentation
  • Guidance on where to get help for payroll questions

PPL

Committed Home Care is a partner to PPL (Public Partnerships LLC), the statewide Fiscal Intermediary designated by New York State for CDPAP payroll and required administration.
